Here at Agape Women’s Specialists, medical records are strictly confidential. Access to your medical records can only be released with your written consent unless required for police investigation.

Please come to our Centre to complete and sign the ‘Consent for Release of Medical Information Form’. For insurance claims, please give us the insurance claims form with the authorisation section fully completed and duly signed.

Payment of S$80-150 is required upon request for a medical report, as regular consultation or hospitalisation fees do not include cost for a medical report. A fee has to be levied for this additional service to cover administrative costs and professional input by the doctor.

*The turnover time for completion of a medical report is about 7 to 14 working days.
